This questionnaire is intended to address the Federal McKinney-Vento Assistance Act. Your child may be eligible for additional educational services, school supplies, school uniforms, parent and student workshops, immediate enrollment to the school of origin, counseling, transportation to and from school, summer enrichment programs, and community agency/medical referrals through Title IX, Part A of the Every Student Succeeds Act. Program eligibility may be determined by completing this brief questionnaire. Information provided on this form is confidential.
