If medication for your child is needed while at school, please bring the appropriate form to the school clinic. In order for medication to be administered to your child during the school day, the medication and a completed Medication Authorization form must be present.

Please help us better serve your child by completing the following health care plan or plans that are specific to your child’s condition and return to the school clinic.
